Over 13% of Tennesseans live with diabetes, but you don’t have to face it alone. With free resources and support, managing blood sugar, preventing complications, and thriving is possible. Let’s take charge together!


1. Tennessee Diabetes Prevention and Control Program The TN Department of Health provides free screenings, nutrition counseling, and education. Call 615-741-3111 or visit TDH Diabetes Resources to connect.


2. CDC’s National Diabetes Prevention Program Join free lifestyle change programs to reduce diabetes risk. Call 1-800-CDC-INFO (1-800-232-4636) or visit CDC Diabetes Page.


3. Local Health Department Support All 95 counties offer free A1C testing and diabetes classes. Contact your county office (e.g., Davidson: 615-340-5616; Shelby: 901-222-9000).
